A potent synthetic hexapeptide growth hormone secretagogue. Studies have investigated its strong GH-releasing activity and additional cardioprotective properties mediated through CD36 receptor binding.
Hexarelin is a synthetic hexapeptide GH secretagogue that activates the GHS-R1a receptor with high potency, producing robust growth hormone release from the anterior pituitary. It is considered one of the most potent GHRPs in terms of peak GH stimulation. In addition to its GH-releasing activity, hexarelin binds to the CD36 scavenger receptor on cardiac and vascular tissue, conferring direct cardioprotective effects independent of GH release. This dual mechanism involves both pituitary GH axis stimulation and direct cardiovascular tissue protection. However, hexarelin also produces notable elevations in cortisol and prolactin.
Broglio F, et al. Activity of GH/IGF-I axis in patients with dilated cardiomyopathy. Clin Endocrinol (Oxf). 1999;50(4):417-430.
Key finding: Hexarelin stimulated significant GH release and demonstrated distinct cardiac effects mediated through non-GHS-R pathways, suggesting direct cardioprotective mechanisms.
Bodart V, et al. CD36 mediates the cardiovascular action of growth hormone-releasing peptides in the heart. Circ Res. 2002;90(8):844-849.
Key finding: Identified CD36 as the binding site mediating the cardiovascular actions of growth hormone-releasing peptides in cardiac tissue, distinct from the GH secretagogue receptor.
